We are hiring Care Coordinators (CC) also known as BHTS, MHTs, Peer support, to manage and maintain the safety of the patients. Care Coordinator's are viewed as the heartbeat of a multidisciplinary team that provides direct inpatient care to adults 18 who are challenged with mental health and/OR substance use disorders.

• Building rapport with patients, clinical staff and medical staff.
• Work as a One Team Mindset
• Transporting patients to and from any outside appointments when needed
• Observing patients and ensure safety while in treatment.
• Assist clinical and medical staff with intakes, orientations and daily operations.
• Assisting in Self-Administered Medication
• Conducts and documents individual's vitals and reports all questionable results to medical staff. Conducts and documents all BAC and urine drug screens
• Documents all services and significant individual interactions in the electronic health record. Monitors and documents patients every 15 mins
• Responsible for maintaining and ensuring the cleanliness of the facility. Performs light cleaning of interior center, restocking and launders individuals clothing when needed
• Facilitates group sessions daily to individuals
• Demonstrates positive role modeling for the clients through mentoring and coaching while utilizing recovery skills

Pay: $16.00 - $18.00 per hour
